Instruction and Outreach Committee (IOC)
TEMPLATE: Feedback/ Class Evaluation Form

The instruction & outreach coordinators of IOC are interested in developing a systematic method of measuring effectiveness of
instruction efforts within the UCSD libraries.  The instruction coordinators agree that evaluation/feedback forms should be used
for all library instruction classes (including course-integrated instruction conducted by bibliographers).

Instructors are asked to use the following template consisting of (8) core evaluation questions and a section for comments.
At the end of the class, please send a copy of the forms or the results to Amy Butros, mailcode 0699.
